If you choose to submit a postal reclaim to us, copies of required supporting documents will need to be certified by a relevant professional. More information about certification can be found in the FAQ above.
The documents we will require are as follows:
Proof of Identity (one of the following)
- Copy of current valid Passport or
- Copy of current valid Driving Licence
In circumstances where the original account or product name and the name held on the Proof of Identity document do not match, evidence in respect of name change is required, such as;
- Copy of Marriage/Civil Partnership Certificate or
- Copy of Name change order from Court or
- Copy of Deed Poll
Proof of Address (one of the following)
- Copy of current valid Driving Licence (if not used as Proof of Identity) or
- Copy or original copy*of utility bill/statement dated within the last six months from a regulated utility provider e.g., Gas/Electricity/Landline telephone or
- Copy or original copy*of Bank Account Statement dated within the last six months from a regulated Bank/Building Society/Credit Union (can include a Mortgage Statement and must be less than 12 months old)
*please note original documents can not be returned.
Proof of Destination Bank Account
Copy of Bank Account Statement confirming the Bank account you wish funds to be sent to where a reclaim is successful.
Note: The bank statement must have been issued within 6 months of receipt of a reclaim form, mobile banking app screen shot is not sufficient). The details (i.e. address) of the statement must match the documents you are providing for address verification purposes. This bank statement can also be sent to us for your address verification.
Proof of PPS Number
We require confirmation of your PPS Number, a copy of one of the following documents can be used to evidence this:
- Public Services Card or Social Services Card
- Drugs Payment Scheme Card
- Medical Card or GP Visit Card
- European Health Insurance Card
- Letters from the Department of Social Protection (e.g. Social Welfare payment slip)
- Letters from the Revenue (e.g. PAYE Notice of Tax credits, Tax Assessment, etc)
- Temporary Payment Card
Proof of UK National Insurance Number
We require confirmation of your UK National Insurance Number, a copy of one of the following documents can be used to evidence this:
- P45 or P60.
- Letter from HM Revenue and Customs about tax or tax credit.
- Bank statements showing payments by direct debit for class-2 National Insurance contributions, or benefit payments received showing your National Insurance number on the statement, for example, Incapacity Benefit or State Pension.
- Payslips, salary statement or works pension statement (as long as it shows your National Insurance number).
- Letter that the Pension Service or Jobcentre Plus has sent to you (not handwritten).
- National Insurance number card (not handwritten).
Proof of other Foreign Tax ID Number
If you don’t have a PPS Number or UK National Insurance number, evidence of another tax identification number will be required.
Deceased Beneficiaries

We will require the following documentation to support a reclaim on behalf of a deceased beneficiary. We will advise you of what is specifically required for your reclaim.

- Certified copy of death certificate, interim death certificate or coroner’s certificate.
- Proof of PPSN or Tax number for the deceased beneficiary (documents listed above).
- Certified copy of Proof of Identity and Address for all Executor(s) or next of kin.
- For total Estate value less than €50k, Small Estates Indemnity Form signed by all Executor(s) or next of kin.
- For total Estate value greater than €50k Certified copy of Grant of Probate/Letters of Administration.
Proof of Ownership of Account
In certain situations, we may require one of the following documents in relation to your closed Ulster Bank Ireland DAC account/product:
- Copy of Bank Account or Product Statement or
- Copy of Ulster Bank Ireland DAC letter which notes Account or Product reference Number, Account or Product Name and Address or
- Copy of physical passbook which notes Account Number, Account Name and Address or
- Copy of a Deposit Receipt or
- Copy of Debit/ATM Card or
- Copy of a Cheque Book
- Copy of original draft or indemnity form
